The HLSS is funded via the following programs: 1. The Home and Community Care Program (HACC) Through HACC program funding, a number of basic support services are provided, with the aim of assisting frail older people and younger people with a moderate to severe disability to remain living at home, thus preventing inappropriate or premature admission into residential care. Carers of people who are frail aged or younger people with a disability are also assisted by the HACC Program, for example by being provided with time out from their caring role in the form of respite care. HACC services provided directly by the HLSS are: Transport - to doctor's surgeries or other professional appointments, shops, the bank, hairdresser, etc. Some of this transport is provided utilising a small mini-bus or by taxis. Respite Care - providing a substitute carer to enable the full-time carer of a frail older person or younger person with a disability to have a temporary break from their caring role. Case Management - planning, monitoring and reviewing a range of support services for people who have complex care needs. Home Maintenance and Modification - assistance to maintain, modify or repair a home, making it safer and easier to remain living there. Social Support - periodic full or half day Mystery Trips are provided, particularly for people who are socially or geographically isolated. Information - providing brochures and information on a range of topics related to ageing, disabilty and caring. Referral - to other services such as the Aged Care Assessment Team, Meals on Wheels, Community Nursing, Home Care. etc. 2. The Community Transport Program (CTP) This program, funded by the Ministry of Transport, aims to assist members of the community who are transport disadvantaged. Transport assistance can be provided to attend medical or other professional appointments, usually in Canberra, Queanbeyan or Goulburn. Direct transport to Sydney is not usually provided. 3. The Disability Services Program (DSP) This provides the HLSS with some additional funding specifically to provide carers of younger people with a disability flexible options for respite care.
